Issuance of Business Permit

CHECKLIST OF REQUIREMENTS	WHERE TO SECURE
BUSINESS OWNERS 1 original copy of Barangay Clearance 1 original copy of Barangay Resolution for: - Pisonet/Internet Cafe - Mahjong - Billiard Table - New Beach Resort - Videoke - Game Fowl Breeding/Poultry	Barangay clearance/Resolution will be secured at the barangay where the business establishment is located
1 original copy of Cedula	Cedula will be secured at the barangay where the business establishment is located or it can be secured from the City Treasurer's Office
2x2 Picture for new Applicants	To be secured by the Business Owner
Sketch map for new Applicants	A sketch map of the business establishment is secured by the applicant
Income Tax Return for business renewals	ITR is secured from the BIR an to be presented during the renewal of business permit for assessment
BEACH RESORT Foreshore Lease Agreement (FLA) Environmental Compliance Certificate (ECC) Waste Water Discharge Permit Permit to operate generator set	To be secured from the Department of Environment and Natural Resources (DENR).
Department of Tourism (DOT) Annual Certificate of Accreditation (All Tourism Related Business)	To be secured from the Department of Tourism (DOT)
WATER REFILLING STATION Water Source: Deep Well Barangay resolution Interposing no objection SP Resolution Interposing no objection	Barangay resolution is secured from the barangay where the business establishment is located and the SP resolution is secure from the Sangguniang Panlungsod Office if the source of the water is from deep well.
Water Source: Deep Well Water District Certification	Water District certification is secured from the Water District Office if the source of the water is from Water District.
Water Source: Barangay Water System Barangay Resolution	A Barangay Resolution is secured from the Barangay where the water refilling station is located and connected.
BAY AND RIVER LICENSE for passenger boat and tourist water transport vehicles	Bay and river license is secured from the MARINA Office
IF PROCESSING OF APPLICATION IS DONE THROUGH A REPRESENTATIVE Special Power of Attorney (Sole Proprietorship) Secretary's Certificate/Special Power of Attorney (Corporation, Partnership) Secretary's Certificate/Board Resolution (Cooperative)	Special Power of Attorney (SPA)/Secretary's Certificate is secured from the proprietor of the Company who owns the business establishment. Secretary's Certificate/Board Resolution is secured from the cooperative.
ACKNOWLEDGEMENT OF REGISTRATION (AOR) OR AUTHORITY TO OPERATE (AO) AND CERTIFICATE OF REGISTRATION (COR) FROM CENTRAL BANK (Pawnshops, Foreign Exchange Dealers, Money Changers, Remittance Agents)	Acknowledgement of registration (AOR) or authority to operate (AO) and certificate of registration (COR) is secured from the Central Bank if the business establishment is a Pawnshop, Foreign Exchange Dealer, Money Changer and Remittance agents.
SECURITY EXCHANGE COMMISSION (SEC) REGISTRATION, ARTICLES OF INCORPORATION & BY-LAWS (Corporation, Banks and other Financial Institutions)	Security Exchange Commission (SEC) registration, articles of incorporations & by-laws is secured from the Security Exchange Commission Office if the business is owned by a corporation.
COOPERATIVE DEVELOPMENT AUTHORITY (CDA) REGISTRATION, ARTICLES OF INCORPORATION & BY LAWS (Cooperative)	Cooperative Development Authority (CDA) registration, articles of incorporation & by -laws is secured from the Cooperative Development Authority Office if the business is owned by the Cooperative.
CONTRACTOR'S LICENSE (General Engineering Business)	For General Engineering Businesses, Contractor's License must be presented during application.
BUREAU OF FOOD AND DRUGS (BFAD) LICENSE TO OPERATE (Sellers of Drugs & Related Business, Food Processor)	BFAD license to operate is secured from the Bureau of Food and Drugs Office if the business line is selling Drugs & Related Business and Food Processors.
DEPARTMENT OF EDUCATION (DepEd) ACCREDITATION (Schools)	For educational institutions and schools, DepEd Accreditation is secured from the DepEd Office.

BUSINESS CLEARANCE
City Environment and Natural Resources City Treasurer's Office City Planning and Development Office City Engineer's Office City Health Office City Investment and Tourism Office City Veterenary's Office			After the interview for paperless application for new and assessment of gross sales for renewal, Business clearances are secured from the regulatory offices in the LGU.
